Do you have any dependencies set? e.g. More in cavity. Is your Opacity of the brush set really low? Have you locked your layer's transparency? (apparently you can still fill a layer even if the transparency is locked) Which brush tool do you have activated? Do you have Colour selected as your channel to paint?
