There's a fine thread in zbrushcentral forum. Please read it carefully, especially #post29. Its critical for great results IMO. I tested it.

How can we handle this using 3dC? I own zbrush, so I'm using the 3dC>zb>blender workflow. But now new blender2.5 alpha2 build can handle millions (40) of poly in sculpt mode (multires). So its possible for someone to do this with 3dc and blender only.
